AISD Donation Receipt/Grant Award Notification

This form is to be completed by staff upon receiving a donation or grant on behalf of any Austin ISD campus or department. Once submitted, you will receive a confirmation of the submission which you may print for your campus records or provide to your donor as a tax receipt.
